Last night I went to a gay video store to get a blowjob through a glory hole. But the guy on the other booth decided to insert my bare penis into his anus instead without warning. Luckily I felt something was off and pulled out immediately. I think the tip of my penis might have been inside his rectum and I might have been inside no more than 4 or 5 seconds.

Should I start getting PEP to prevent infection? Should I get tested in a few weeks?

From the information given, this scenario is determined to be High Risk (evidence of transmission through these activities and is the majority of cases of transmission).

Although the exposure was very brief, the high level risk remains since the three components of the transmission equation were met. Specifically, rectal secretions can contain the HIV virus, which could gain direct access to your bloodstream through the tip of your penis during an unprotected penis/anus exposure. Although the risk is high, it is important to note that the individual receiving anal sex is 13 times more likely to acquire HIV than the individual who is the insertive partner (1).

If you are continuously engaging in high risk activity there are biomedical approaches such as PrEP and PEP that help lower your risk of acquiring HIV. Refer to your physicians for further information.
