Hi there and thank you for contacting AIDS Vancouver, we are happy to help.

It sounds like you are concerned about your risk for HIV transmission after pricking yourself with a safety pin which may have been pricked into someone else 4-5 hours prior.

This is a very good question, because as we can see from the transmission equation posted below, sharing needles does have the potential for HIV transmission. However, the needles that pose a risk are ones which are barrelled, ones that have a hollow opening which holds air/blood inside. A safety pin is NOT barrelled and therefore does not pose the same risk. This is due to the principle that once a bodily fluid containing HIV is exposed to air, it is no longer able to transmit HIV.

HIV TRANSMISSION EQUATION

BODY FLUID
+
ACTIVITY
+
DIRECT ACCESS TO BLOODSTREAM
---------- -------- ----------------------------
• blood (including menstrual) • unprotected anal or vaginal intercourse • vagina
• semen • sharing needles • anus
• pre-cum • mother to child (in specific cases) • urethra in the penis
• rectal secretions • open cuts and sores (in theory)
• vaginal fluids • other mucosal membranes
• breast milk • points of needle injection

In the scenario that you described, once someone pricked their finger on the safety pin, the blood would immediately be exposed to air on the safety pin, and therefore not be able to transmit HIV. Therefore, when you were to prick your finger on the same pin a few hours later there would be NO RISK, since the blood is not able to transmit HIV anyways.

Hopefully this information was helpful to you, and if you do have any further questions or concerns, feel free to contact us at our helpline.

Sincerely,

Raveena
