Hello,

Thank you for your inquiry. From what we gather from the question, you were asking about the conclusiveness of your non-reactive P24 Combo and Western Blot tests 93 days post exposure.

Please consider the following:

The P24 Combo test is most commonly used to detect the antibodies provoked by the presence of the HIV P24 protein as well as the protein itself. This protein is detected ONLY within the first few weeks of infection. The P24 Combo test is considered to be among the 4th Generation tests in which most HIV specialists agree that these tests can be considered conclusive at 6 weeks. However, official HIV testing guidelines recommend re-testing at 12 weeks with an antibody test (1). In this case, the negative result of your P24 test (within the recommended time frame of testing) would have been considered highly reliable, yet it was responsible of you to seek further testing for additional confirmation.

Western Blot testing can be considered the "gold standard" of HIV confirmation testing. This test has a window period between 4-6 weeks and can be considered conclusive after this period. Western Blot has an extremely high accuracy in detecting HIV antibodies, which is why it is used as a standard protocol for HIV testing confirmation (2). In your case, if you received the Western Blot more than 6 weeks post exposure, it is safe to presume that your non-reactive results can be considered conclusive.

Recommendation: There is no need for additional HIV testing with the scenario provided, please refer to a physician for other health related questions.

All the best,
AIDS Vancouver Helpline/Online, Cody
