please let me know whether HIV transfers when penetrating penis into vagina through underwear

I am kind of confused about this question, as I can see what you're saying two ways: either the penetration was through the hole in the underwear, or the penetration was actually including the underwear.

Regardless of which case, this activity still poses a high risk, since underwear isn't a protective barrier, not like a condom.
