Dear friends

One day ago, a sex worker gave me one second unprotected oral sex. There was some one his saliva present. I immediately wore a condom then he sucked my penis protected for less than 10 seconds. He also licked my nipples. I used a light to see his mouth condition, which was generally good. However there was a tiny blood dot under his tongue. I am not sure whether it was bleeding when he sucked my penis.

This guy was negative few months ago but tested positive today. Should I take PEP or is this a risk for HIV ?

It sounds like you are concerned about the risk of transmission from oral sex since finding out that your partner is living with HIV.

We cannot tell you whether you should take PEP, that remains your choice, however, it would be unlikely that a healthcare provider would prescribe PEP for a negligible risk encounter.

Please be reassured that receiving oral sex is a negligible risk activity, meaning that there has never been a confirmed report of transmission in this way. Saliva is not one of the body fluids that carries HIV, and in fact, inhibits transmission through damaging the virus. As you say that your partner had generally good oral hygiene, it is unlikely that there would have been blood present, and even then the amount would need to be significant.
